You could check your credit card or PayPal activity by searching for a payment of $25, $50, $100 or more to "MVC" or "MI Vax Choice." You can view the membership levels here . If you are certain that you have signed up previously and paid for membership on this website previously, you can login to check membership status . If you do not have an account to login to this website, it could be possible that you donated via our Breeze giving platform , which is located here. If that platform looks familiar to you, please check your credit card or PayPal activity, or submit a technical support request and a volunteer will look into the information for you. I want to change my credit card before my renewal. Is that possible? If so, how? Yes, you can update your payment information prior to your renewal. Please login and go to My Wallet . My membership was canceled, but I didn't cancel it myself. What happened? If the credit card on file was expired or no longer valid, your membership renewal will be automatically canceled. The system does send out email reminders leading up to your renewal and email notifications informing you of a failed payment. Those reminders and notifications may have gone to your junk or spam folder and are easy to miss. In both cases, you would submit the Authorization To List My Practice form . Please note if your Professional membership lapsed, the listing will be taken down, and will be added again upon renewal if permission was previously provided to list your practice. Professional members are emailed automatically by our system informing them of this benefit/option, but it is possible that the email may have gone to your junk or spam folder. Thank you for being proactive about updating us about your new contact information. You can access and update your contact information here . The cost of my membership payment went up from the prior year. What happened? We have not changed our membership pricing in several years. We have only added lower price points to assist with the tough economic conditions. During the year of 2025, we did offer a 30% discount on new memberships for the first year only. This was in celebration of MVC turning 30 years old. Therefore, if you signed up at the Friend level in 2025 with the discount, you would have paid $17.50, but in 2026 you will pay $25 for your renewal. Likewise, if you signed up at the Supporter level in 2025 for the first time with the discount, you would have paid $35, but in 2026 you will pay $50. New Defenders would have paid $70 in 2025, and $100 in 2026, and so on for the other levels. Please note that MVC does reserve the right to alter membership prices at any time, but will provide members with ample notice prior to renewal. Do you send an email notification about my renewal prior to my card or PayPal account being charged? I am a paid member, but have not received the MVC newsletter yet? The newsletter is sent via email once per month, sometimes more frequently if there are important action alerts or upcoming events. If you are member, there may be a short delay to get your information added to our mailing list. MVC does not typically offer refunds for paid memberships unless there was an obvious issue or error. Occasionally, a member will sign up twice not realizing they already have a paid membership, and we are very happy to refund those situations. We can purge your information from our systems upon request. Please allow up to 30 days for your request to be completed. If you were ever a paid member or donor, we may need to keep some identifying information by law, but we will remove all contact information that we have and archive your profile in our member records. Michigan Statute §333.9215 Personal Exemptions MCL 333.9215 (2) “A child is exempt from this part if a parent, guardian, or person in loco parentis of the child presents a written statement to the administrator of the child's school or operator of the group program to the effect that the requirements of this part cannot be met because of religious convictions or other objection to immunization.” Medical Exemptions MCL 333.9215 (1) “A child is exempt from the requirements of this part as to a specific immunization for any period of time as to which a physician certifies that a specific immunization is or may be detrimental to the child's health or is not appropriate.” "Okay, but what does this mean for me?" Michigan State Law establishes that a parent can secure a vaccine exemption for their child from the requirements of mandatory vaccination by presenting an appropriate written statement to their child’s daycare or school. Yet under the Family Educational Rights and Privacy Act (FERPA) , a written statement provided directly to a school becomes part of the student’s school record. Unless narrow exceptions apply, a student’s record is not subject to routine disclosures—including the health department—without a parent’s express permission. Admin Rule 325 176 "What about the health department waiver?" Michigan was the first state to mandate an "education session" at a local health department when they enacted Administrative Rule 325.176 (12) on January 1, 2015, requiring parents and legal guardians who want a religious or philosophical exemption to attend an “education session” and obtain a certified waiver. Parents must provide private information about their child at the session, often against their consent, sign statements with which they might disagree, and answer questions with which they are not comfortable. As a result, some members of MVC have opted to provide their own statement pursuant to the law . Issues Parents & Guardians have with Administrative Rule 325.176 (12) Directly conflicts with Statute §333.9215 Feeling bullied and discriminated against Parents who have attended these sessions have had their religious beliefs questioned and felt harassed and discriminated against. Under an undue burden A separate "education session" is required at multiple school checkpoints and for each child in the household—up to six sessions per child (e.g., one session per year of daycare, pre-k or kindergarten, and 7th grade). So, for twins, under the Administrative Rule, a parent must attend 12 separate sessions. For 3 children, 18 sessions, and so on. This is an extreme burden on parents and caregivers, regardless of their financial means, access to transportation, work schedule, and other factors. Bearing that in mind, we must ask how many healthcare providers adhere to Federal Law to provide a VIS to vaccine recipients prior to vaccination, and is there any consequence for this breach in trust? Unlike the simplified VIS, the package insert is a highly detailed multi-page document with important information on dosing, contraindications, safety trials, and post-marketing observations if available. Download the MMRV package insert (pdf) Before You Vaccinate Prior to the appointment, prepare any questions you may have for your healthcare provider about ingredients , efficacy, and storage requirements of your vaccine to ensure they were met (some vaccines need to be refrigerated). At the appointment , your provider is required by Federal Law to give you a Vaccine Information Sheet (VIS) prior to administering the vaccine. Review the VIS with your provider. If the vaccine is coming from a multi-dose vial, ask your provider if the bottle has been properly shaken prior to each draw, and which recipient or dose number you would be considered. Before your appointment concludes , document the following additional details for yourself.
